Nozzle Blew Off The End Of The Lance And Will Not Stay Attached

This is a Turbo lance nozzle problem. It occurs when the nozzle is not screwed on to the lance tight. The lance with have to be replaced because the threads on the lance are stripped once the nozzle blows off.
